Regulations for the Acquisition, Use, and Refund of Vouchers and Deposits

IDenta Centrum Dentystyczne Sp. z o.o.


1. General Provisions

  • These regulations define the rules for the acquisition, use, and refund of vouchers and deposits offered by Identa Centrum Dentystyczne Sp. z o.o.

  • The purchase of a voucher or the placement of a deposit constitutes full acceptance of these regulations.

  • The regulations are available on the clinic's website and at the reception desk.

2. Definitions

  • Voucher — a document confirming a prepayment for future dental services at the Identa clinic.

  • Deposit (prepayment) — an amount paid toward a future service.

  • A voucher and a deposit do not constitute a down payment (earnest money) within the meaning of the Civil Code unless otherwise stated in writing.

  • A voucher is not a fiscal receipt or an invoice.

3. Forms of Acquisition and Payment

A voucher or deposit can be purchased:

  1. Directly at the clinic (cash / card / BLIK).

  2. Online (if such an option is available).

Important: When paying in cash, the client receives a fiscal receipt; the safekeeping of this receipt is the sole responsibility of the client. The clinic does not issue duplicates of fiscal receipts.

At the client's request, the clinic may issue a payment confirmation, which:

  • is not a fiscal document;

  • does not replace a receipt;

  • does not entitle the holder to a cash refund.

4. Right of Withdrawal (14 Days)

  • The right to withdraw from the contract within 14 days is granted only in the case of a voucher purchased online or remotely, in accordance with consumer protection laws.

  • This right does not apply to vouchers purchased directly at the clinic.

  • In the event of an effective withdrawal from the contract, the client is obliged to return the unused voucher.

5. Refunds

5.1 General Rules

  • Refunds are possible exclusively in cases provided for by these regulations.

  • The clinic reserves the right to refuse a refund in cases not covered by the regulations.

5.2 Cash Refunds

  • A cash refund is possible only upon presentation of the original fiscal receipt.

  • The absence of a fiscal receipt strictly precludes the possibility of a refund, regardless of the circumstances.

  • Refunds are made using the same payment method used for the purchase, whenever possible.

6. No Right to a Refund

The following are non-refundable:

  • vouchers purchased directly at the clinic;

  • cash payments without the presentation of the original fiscal receipt;

  • partially used vouchers;

  • vouchers after the expiration date;

  • lost or damaged vouchers.

7. Voucher Validity Period

  • The voucher has a validity period determined at the time of acquisition.

  • The voucher's validity period is not automatically extended.

  • Extension is possible only by individual agreement with the clinic and is not its obligation.

8. Change of Voucher Purpose

A voucher may be:

  • used for a different service;

  • transferred to another person;

  • This is carried out by prior agreement with the clinic.

The clinic reserves the right to refuse a change of voucher purpose in justified cases.

9. Client Responsibility

The client bears full responsibility for the safekeeping of:

  1. The voucher.

  2. The fiscal receipt.

The clinic is not responsible for the consequences of the loss of documents by the client.

10. Legal Basis

A cash refund without the presentation of the original fiscal receipt is impermissible under current tax legislation. A clinic maintaining a fiscal cash register is not entitled to issue refunds based on copies of documents, client statements, or internal payment confirmations.

11. Final Provisions

  • The clinic reserves the right to amend these regulations.

  • In matters not regulated by these regulations, the provisions of Polish law shall apply.

  • The regulations enter into force on the day of their publication on the clinic's website.
