Wisdom teeth are the last to come through, usually between 17 and 25, and in some people never. There is often not enough room for them, so they grow at an angle, erupt only partly or stay buried in the bone. Even so, wisdom tooth removal is not something everyone needs.

A healthy, straight wisdom tooth can stay for life. Trouble starts when the gum around it keeps getting inflamed or the tooth pushes against its neighbour.

How do you know a wisdom tooth is a problem?

Some signs you will notice yourself, such as pain at the back of the jaw that spreads to the ear, or a bad taste in your mouth. Others only show up on an X-ray: a tooth stuck in the bone, lying on its side or pressing on the root next to it. That is why we decide on removal only after an examination and an X-ray.

iDenta dentist's tip: Many people come in only after the tooth has hurt for days and the cheek has swollen. Acute inflammation often has to be settled first, with the tooth removed later, so the whole process takes longer. If the gum behind your last tooth starts to hurt when you bite, book a visit straight away.

When a wisdom tooth should come out

The most common reasons. The dentist decides after an examination and X-ray.

  • Gum inflammation that keeps coming back

A partly erupted tooth leaves a flap of gum over it. Food and bacteria collect underneath, and this pericoronitis tends to flare up again and again.

  • Impacted or tilted tooth

Lying sideways or trapped in bone, it can damage the root of the molar in front.

  • Decay

Wisdom teeth are hard to clean. Either they decay or the tooth in front does.

  • Pain or a stiff jaw

Pain spreading to the ear or trouble opening your mouth means you should see a dentist now, not next week.

  • Orthodontic plans

An orthodontist sometimes asks for wisdom teeth to be removed if they block planned tooth movement.

  • A cyst

A cyst can form around an impacted tooth. It shows up on an X-ray.

When a wisdom tooth can stay

If it has come through straight, meets the opposite tooth, can be brushed properly and does not hurt, we simply keep an eye on it at check-ups. We don't take out healthy wisdom teeth “just in case”.

How wisdom tooth removal works

Anything from a few minutes to about an hour, depending on how the tooth sits.

  1. Consultation and X-ray

The dentist examines the tooth and uses the X-ray to see how the roots lie and how close they are to the nerve.

  2. Anaesthetic

Local. During the procedure you feel pressure at most.

  3. Removing the tooth

An erupted tooth usually comes out whole. An impacted one may need a small cut in the gum and removal in sections.

  4. Stitches

The dentist cleans the socket and, if needed, places stitches to help it heal faster.

  5. Aftercare and check-up

We explain how to look after the socket. If you had stitches, they come out after 7–10 days.

Healing after wisdom tooth removal, day by day

This is how healing usually goes. If your dentist tells you otherwise, follow their advice.

  • The first 2 hours

Bite on the gauze for about 30 minutes. Don't eat until the numbness wears off, or you may bite your cheek.

  • The first 24 hours

Cold packs on the cheek. No rinsing and no hot drinks, so the clot stays in the socket.

  • Days 2–3

Swelling and pain usually peak, then start to ease. Keep food soft and lukewarm.

  • After 7–10 days

Check-up and stitch removal. The worst is behind you.

  • After 3–4 weeks

The gum closes over. The bone keeps rebuilding for a few months, but you won't feel it.

What to avoid after the procedure

Especially in the first days. These cause most complications.

  • Rinsing on day one

  • Drinking through a straw

  • Smoking

  • Sport and saunas for 3–4 days

  • Chewing on that side

When to see the dentist again

Mild pain, some swelling and pinkish saliva in the first days are normal. Call us if:

  • the pain gets worse after day 3 instead of easing; this may be dry socket, which needs treatment;
  • the bleeding does not stop after biting on gauze for 30 minutes;
  • the swelling grows after day 3, or opening your mouth gets harder;
  • you develop a fever, or notice a bad smell or taste from the socket.

iDenta dentist's tip: Bite down firmly on the gauze and leave it there. Taking it out every few minutes to check whether you are still bleeding only drags the bleeding out. Sleep with your head raised on an extra pillow the first night. A pink mark on the pillowcase in the morning is nothing to worry about.

These complications are rare and respond well to treatment if you don't wait. Call reception on +48 731 771 877. We also treat pericoronitis and dry socket.

Frequently Asked Questions

Not during the procedure. We do it under local anaesthetic, so you feel pressure at most. Pain and swelling can follow for a few days once it wears off, and your dentist will tell you which painkillers to take.
